Registration Requirements
Provisional Requirements
Certificate / Diploma Level- Certified copy of Leaving Certificate from the institutions that are under NACTE.
- Certified copy of either NACTE transcript or certificate for institutions which are under NACTE.
- Certified copy of academic Transcript from institutions that are under TCU.
- Certified copy of Certificate of Secondary Education (CSSE).
- Certified copy of Birth certificate.
- Current Curriculum Vitae (CV).
- Payment made via requested control number from HPRS.

Degree Level- Introduction letter from university.
- Certified copy of Advanced Certificate of Secondary Education (ACSSE) or certified copy of Diploma / Advanced Diploma in Medical Laboratory Science.
- Certified copy of Certificate of Secondary Education (CSSE).
- Certified copy of Birth certificate.
- Current Curriculum Vitae (CV).
- Payment made via requested control number from HPRS.
Enrollment Registration
Certificate Level- Certified copy of Leaving Certificate from the institutions that are under NACTE.
- Certified copy of either NACTE transcript or certificate for institutions which are under NACTE.
- Certified copy of academic Transcript from institutions that are under TCU.
- Certified copy of Certificate of Secondary Education (CSSE).

Full Registration

Full Registration.
Degree Level- Certified copy of University Certificate.
- Certified copy of academic Transcript from University.
- Certified copy of certificate of Internship.
- Certified copy of Advanced Certificate of Secondary Education (ACSSE) or certified copy of Diploma / Advanced Diploma in Medical Laboratory Science.
- Certified copy of Certificate of Secondary Education (CSSE).
- Certified copy of Birth certificate.
- Current Curriculum Vitae (CV).
- Payment made via requested control number from HPRS.
- Internship Logbook.
Temporary Registration.
Certificate/ Diploma / Degree Level- Filled registration form.
- Certified copy of academic certificate from University.
- Certified copy of academic Transcript from University.
- Certified Certificate of recognition of academic qualifications from Tanzania Commission for Universities (TCU) or NACTE.
- Certificate of registration from the registering authority of the last jurisdiction of practice.
- Current Curriculum Vitae (CV).
- Two most recent passport size photos.
- Original payment receipt.
- Certified copy of passport.
Retention Application.
Practitioners with expired Enrollment/ Full- Updated current location in the address menu from the HPRS system.
- Most recent passport size photo with a blue background attached in the profile menu of the HPRS system.
- Payment made via the requested control number from the payment menu in the HPRS system.
Online Examination Application..
Certificate / Diploma /Degree Level- Candidate's provisional registration must be expired or remain with at most three months to expire.
- Candidate must have an account in the HPRS system.
- Candidate should have the payment fee ready to proceed with payment after requesting a control number via the payment menu in the HPRS system.
